We will need the following to process your prescription order.

• Completed Prescription Order Form (Download Here)
• Copy of original doctors prescription that is no more than 2 years old
• PD number - if this is not written on your prescription, contact your doctors office and have this read off your file

Frequently Asked Questions

What is a PD?
PD stands for pupilary distance, which is the distance between the pupils. We cannot complete any Rx without the PD.

Can I e-mail my prescription?
We need a Copy of original Rx with PD and doctor's signature (no more than 2 years old).

How recent should my prescription be?
Florida State Law prohibits us from accepting prescriptions from anyone other than a doctor, hand written without a doctor's signature, or prescriptions older than 2 years old.

When do I need to pay?
Payment is due before start date, there is no exchange or refund once the job has been processed. If you are not sure about the fit of a frame, we can give you the address of one of our retailer or you can be sent few frames to try them on.

How long?
It usually takes 10 business days.

Can any prescription go in any frame?
Ocean Waves lenses can not be put in any other frame than those offered by Ocean Waves. Some can not hold certain prescriptions.
